eStatements are an easy and convenient way to receive your periodic bank statements. Our goal is to provide paperless statements you can view, print and store on your personal computer, when it is convenient for you. You can reconcile your accounts more quickly, reduce the risk of identity theft, plus saving the environment in the process.


By completing the following enrollment form and agreement, you are authorizing United Community Bank to deliver statements of your United Community Bank account(s) by electronic means. You understand and agree that by requesting electronic delivery, you WILL NOT receive statements in paper format. Your consent applies to electronic statements as well as important disclosures or notices that may be presented electronically now or in the future including, but not limited to, notices regarding changes to your account or amendments to any agreements and/or disclosures governing your account. However, you should continue to review any correspondence that we may send you by mail.


Access to eStatements

After choosing to receive electronic periodic account statements and disclosures, you will receive an e-mail from the Bank at the e-mail address you have designated to inform you that the statement is available for viewing. The information will be available on our United Community Bank Online Banking website, www.unitedcommunitybank.com, for at least thirty (30) months from the date the e-mail is sent. Your eStatement may be viewed, using your Username and Password, by clicking on your account (checking and/or savings) and then going to "Documents". eStatements will be viewed electronically in a PDF-Portable Document Format which can be viewed online, saved to your computer or printed at your convenience. Multiple accounts can be registered under one Username/Password as long as the user is an owner on each account registered. The user will be able to view all registered account statements, including accounts held jointly with other account holders. Account statement notification cannot be sent to multiple e-mail addresses. You also agree that electronic delivery will satisfy United Community Bank's requirement to provide you a periodic statement of your account(s) activity.


Eligible Accounts

All checking and savings accounts are eligible for eStatements. eStatements will generally be available at the same frequency (cycle) as previously.


Hardware and Software Requirements

Please refer to the same hardware and software requirements as outlined in the Online Banking Agreement. You will be provided updated information in advance should any changes be made to these hardware or software requirements by notification via your e-mail address and on our website.


Right to Withdraw Consent for eStatements


You have the right to withdraw your consent to receive electronic periodic statements and disclosures at any time. To withdraw your consent to receive electronic statements and disclosures, you must notify us in writing at United Community Bank, Attn: Bookkeeping, P.O. Box 167, Milford, IA 51351. You may also telephone us at 712-338-2781 or 888-338-2781 or notify us in person at any United Community Bank Office. The withdrawal of your consent should be received at least ten (10) days before the end of your normal statement cycle. A monthly $3.00 Paper Statement Fee will then be charged to your account.

  

Obtaining Records in Paper Format

You may receive a paper copy of your statement by contacting United Community Bank by telephone at 712-338-2781 or 888-338-2781 or by mail at United Community Bank, Attn: Bookkeeping, P.O. Box 167, Milford, IA 51351 or in person at any United Community Bank Office. A $3.00 Paper Statement Fee will be charged per statement.


Customer Responsibilities

You are responsible for accessing, opening and reading your statements at your earliest convenience. You are responsible for promptly notifying United Community Bank if any documents you receive are incomplete, unreadable or inaccessible. You must have a valid e-mail address. You agree to immediately notify United Community Bank of any changes to your e-mail address. Please change your e-mail address by clicking on the "Profile" tab on the United Community Bank Online Banking. If an e-mail is returned undeliverable for any reason, we will make an attempt to contact you via phone. Failure to update and/or correct your e-mail address will result in United Community Bank reinstating the delivery of your paper statement. A monthly $3.00 Paper Statement Fee will then be charged to your account. If you currently receive duplicate statements, mailing of the duplicate copy will be discontinued. However, you may print multiple copies and distribute those at your own discretion.


E-mail will sometimes get collected in a junk mail/spam mail folder by your E-Mail/Internet service provider. Please verify your spam filter is not blocking United Community Bank's e-mail.


As a United Community Bank Online Banking customer, you have chosen a Username and Password which allows you access to our Online Banking service. You are responsible for keeping your Username and Password confidential and for ensuring that you have logged out when your session is complete to prevent unauthorized access. It is your responsibility to contact us if you know or suspect unauthorized use of your Username and Password. Review our Online Banking Agreement and Disclosure for further information. You agree to provide true, accurate, current and complete information about yourself as requested, and not misrepresent your identity. You agree that United Community Bank's security procedures are commercially reasonable.


You understand that you have a duty to exercise reasonable promptness in examining the eStatement which includes your cancelled checks for unauthorized signatures, alterations, forgery, posting errors, etc. The statue of limitations governing these responsibilities will commence at the time the Bank sends you the e-mail notification that your eStatement is available. We must hear from you no later than 60 days after we sent you the FIRST statement on which the error or problem appeared. Notify us immediately in writing at United Community Bank, Attn: Bookkeeping, P.O. Box 167, Milford, IA 51351, or call us at 712-338-2781 or 888-338-2781 to report any suspected error, alteration or other irregularity with or unauthorized access to your eStatements.


Privacy

Our privacy policy that has been previously provided to our customers will apply to this service and the policy is incorporated into and made part of this United Community Bank eStatement Agreement and Disclosure. Your e-mail address will be used in accordance with the Bank's privacy statement to deliver notification to you. Your e-mail address will not be sold or otherwise provided to third parties.


Amendment and Termination

 United Community Bank reserves the right to modify this Agreement at any time. Any modifications to this service shall be effective the date provided within the notification posted on our website: www.unitedcommunitybank.com. United Community Bank will notify you of any amendments to this Agreement, including any changes in hardware or software required to access documents by providing notice to you via your e-mail address or our website. Your continued acceptance of electronic disclosures or eStatements after the effective date of such change will constitute your acceptance of and agreement with such amendments(s). United Community Bank has the right to terminate this Agreement at any time and for any reason without advance notice.


Liability

United Community Bank does not guarantee the delivery of any e-mail notification, nor liability for losses or damages arising from non-delivery, delayed or misdelivery. Factors affecting these e-mail notifications are solely between you and a Third Party that you designate, such as an Internet Service Provider and phone company. We make no representations or warranties whatsoever with regard to Third Party Service Providers products or services. You understand that you have no expectations of privacy if the statements are transmitted to an e-mail address owned by a third party, such as your employer. You further agree to release United Community Bank from any liability if the information is intercepted or viewed by unauthorized parties at your employer or other e-mail address selected by you. United Community Bank makes no warranty of any kind, express or implied that our eStatement delivery will be uninterrupted or error free. We do not and cannot warrant that United Community Bank will operate without error, or that eStatements will be available at all times. Circumstances beyond our control (such as, but not limited to, fire, flood, interruption in telephone service or other communication lines, interference from an outside force, acts of God, legal restriction or delays in mail service) may prevent proper delivery of notifications and services. You agree that neither we nor our suppliers or our directors, officers or employees will be held liable for any technical, hardware or software failure of any kind, any interruption in the availability of our service, any delay in operation or transmission, any incomplete or garbled transmission, computer virus, loss of data or other similar loss. To the extent we may have breached any term of this consent and agreement, you agree that your sole remedy is to discontinue use of this service. You also agree not to alter or use the eStatements for any unauthorized, fraudulent, or illegal purpose.

 

Governing Law

This eStatement User Agreement and Disclosure shall be governed by the laws of the State of Iowa.

 

Acceptance

By accepting the terms of this Agreement, you are indicating that you have read, understand, and agree to be bound by this Agreement and that you, and any authorized signers or users authorize us, at our discretion, to provide account disclosures and account statements electronically, until this Agreement is terminated or discontinued in accordance with this Agreement. You also confirm that, in accordance with the "Hardware and Software Requirements" outlined in the Online Banking Agreement, you have equipment that provides the ability to receive and retain electronic disclosures and statements. You also understand that you will no longer receive a periodic paper statement. Please print a copy of this eStatement User Agreement and Disclosure and retain for your records.
